Cos it's nicer and smoother to blip the throttle on a down change and, especially on track, sometimes the ideal time to change down is when you're turning into a corner on the brakes.
wear some work boots - works for me!
pedals are a bit badley spaced - if i've got go kart boots on i cant heel n toe in it at all (i can fit my size 10s between each pedal!)
do a search on US 911 sites - 911's have hinged pedals and the you can definately get extension plates (extensions are illegal in the uk for the road i think however?)
