Labels - Stickers required - Anyone?

I know we have a couple of printers on here, I am after some stickers, labels, standard size I believe, 19mm by 63 mm.

I would like it split into 3 boxes in each box the following:-

LEFT Box:- The CE mark in with the writing KEEP THIS INFORMATION FOR FUTURE REFERENCE underneath

MIDDLE Box:- Quack Quack Consultants Ltd - 29 PL36QP

RIGHT Box:- IMPORTANT: Not for use by children under 3 years old due to possibility water squirting in face, CAUTION do not point or use at FACE or EYES

I am looking for a 1st run batch of 5000 on White Vinyl stickers / labels

Best label printers I know are Adare - www.adare.com - but I'm not sure as to how competitive they are on lower quantities. Very very good for fancy fold-out labels.
