Under caution for DW/OD&C????

I crashed years back (wasn't paying attention - rear ended car in front) and was cautioned in the same way. If my memory serves me correctly they had six months do decide whether to prosecure me or not.
I'd completely forgotten about it and months and months later I got a note saying they weren't going to take any action.

I work in insurance.
There is usually a catchall phrase on the proposal form which says "You are reminded that it is essential you provide all material information likely to influence the acceptance and assessment of this insurance." Telephone brokers and direct insurers often don't have a proposal form they just send you a statement of facts for you to check and with statement including similar wording to the above.
If they ask you if you have any prosecutions pending and you have not received a summons or notice of intended prosecution then I'm not certain how you stand and would normally recomend that you disclose that you have been stopped, however you could take the view that you do not have any prosecutions pending as you have not been sent anything suggesting that there will be and until you do then you have a right to assume that nothing will happen.
If in doubt disclose it and leave it to the person dealing with the quotation to decide if it is pertinent.
