Renewing provisional licence?

My wife started to learn to drive whilst she still lived at home and had her maiden name.

Her provisional expired in 2010 but now she wants to learn to drive again, the options on the form however are slightly vague.

Does she need to apply as "A first provisional licence" seeing as her name and address are incorrect too and she is basically starting again or does she need to tick all boxes that apply, ie. change of name, change of address and renew the photo?
