DVSA have lost my test record

I have a 35 year old lorry that I've had since 2004. It's been SORN since 2013, I used, and tested it, most years prior to that.
I failed the first test and yesterday went for retest and passed. But they couldn't find the vehicle on the DVSA 'system', after two phone calls I was sent away without a pass certificate and told to fill out a VTG1
This form is for a first test, which it self evidently isn't. The form also says I face a heavy penalty on conviction for a false statement.
It seems obvious that the error is with the DVSA records, DVLA records are correct.
Filling in a form incorrectly doesn't seem the right way to resolve this, quite apart from the additional time and cost of a retest.
Not sure what to do.

I phoned them is Swansea today, they can now see the 'technical history' but it's not attributed to the chassis number.
The issue seems to be that at some time they've added R1114 to the original short chassis number. R1114 is the model of chassis.
Strangely the lady on the phone said the tester had said he didn't retest yesterday. I took it myself, watched him go in the pit and him say he was happy with the repair. The issue arose when he tried to log the pass on their system. He said it had passed. I think they're trying to back peddle on this, as they don't think they should have tested it.
I was given an email address for the 'HGV tech' and I've sent them an email. They don't take phone calls. I've sent photos of the chassis number and Ministry plate.
I've said, politely, that I've presented it with the right numbers, everything on the vehicle matches, they've passed it and I would like the test certificate.
I live in hope.

It seems resolved. I called the Road Haulage Association and within an hour I had an email from the DVSA saying they will give me a test certificate and amend their records, rather than amend the lorry to suit theirs.
I must admit I wasn't expecting common sense, so pleasantly surprised.

Yes Oakley bought a new rolling chassis and built the horsebox on top. It is an old design, sharing many parts with ford D series and Thames Trader.It's a bit like driving a Portakabin, but great when you get there.
New Ministry plate arrived today.
