NIP form, are they all like this?

So an NIP drops and not disputing it but what on earth is going on when it can be so badly designed?

It says you must put a cross in one of the boxes below

Poor isn't it. I hate those forms that ask you to circle your response and then presents paragraphs of text, or don't specify whether to tick or X the boxes, or ask for block capitals then want an e-mail address etc. etc.

I note that they still haven't introduced option3, i have never heard of the vehicle identified above

email addresses aren't case sensitive (i'm guessing that's why the comment anyway .. )
