What to fill in on the NIP

The NIP is going back unsigned. I know the name and address need completing, but do the other fields have to be filled in?

1. Driving Licence Number
2. Date of Birth
3. Today’s Date

I filled it all in ..then dont sign it

IMO todays date is the only thing to fill in to be reasonable.

Driver Number - is there to give some sort of linkage to you as a driver and DoB also enables Driver Number to be accessed. BTW do not fill in the Driver Number on a car transfer - its another try on - the notes on the back admit ists optional.
