It does vary - a lot of bigger companies are self insured. They pay a bond in but use an insurance firm to deal with claims.
This means if found at fault the company pays out of it's own resources. So in effect they can stipulate their own insurance regulations.

My own company limits it to driver and spouse/partner and a son/daughter would not be insured except as below.
In principle anyone else not employed by us can be approved on a case by case basis subject to a check.
Anyone employed by the company in possession of a full licence is entitled to drive one of the cars. Eg company employee from say USA / Asia / EU wherever is insured.
Of course if they are working here longer than the DVLA rules allow then they have to take a test. Some fail quite a few times which IMO says something about the UK test as these people are definitely not thickos.
Finally everyone who is issued with a company car and regular users gets a defensive driving course from a specialist training company who use police instructors.
FWIW that's our policy anyway.

My Dad's policy is that anyone (eg Son/Daughter) over 25 can drive the car but they need the owners permission. Now is the owner of the car the company that my Dad works for or the company they lease the car from.
I had a quick look at my Dads policy a while ago and IIRC it doesn't name any individual on it just the company that he works for.

At a guess would say the permission is needed of the company named on the insurance form.
We have a driver's book issued to everyone, recently revised to take account of the mobile phone laws coming in on Dec1.
Maybe a bit bureaucratic but it definitely saves on insurance costs. No exceptions on the check either, eg a relative who is class 1 traffic CI still had to have a check to get on the list to drive my car.
