Most difficult trophies / achievements?

What do we reckon, my list would be (out of the games I've played)
Wipeout HD - Zone Zeus
Wipeout HD - Beat Zico
Trine - Better than the developers (reach top of last level without dying on Very Hard difficulty)
Vanquish - Can't remember what it's called but you get it for beating all the challenges in challenge mode. Challenge 4 & 6 are just about near impossible.
Chime Super Deluxe - Overachiever. I was pretty pleased when I got this one, hardest trophy I've got yet.

Definitely "Dead Men Rising" from Kane and Lynch 2: Dog Days. Complete the game on insane mode, even doing it co-op for the "End of the Road" achievement with my brother who is as much of a gaming ace as me, it was hard as nails.
Terminator-hard enemies who can soak up silly amounts of damage (30+ assault rifles rounds to the chest, what body armour are those guys wearing!), are super accurate and have infinite ammo, whereas you can take literally two bullets before dying, often get pinned in cover and run out of ammo trying to fight back. There is a section on the last level where you and your partner decide to split up - totally nonsensical since fighting together is all that kept them alive, and that was hilariously hard.
Coupled with a bug that would crash the game at the end of a coop level, meaning the whole thing had to be done again, made this a real blood, sweat and tears achievement. It will take a real son of a b
h of an achievement to ever take my "hardest achievement" badge from "Dead Men Rising", but I think that will remain my proudest achievement for ever.
However, I am hooked on insane modes now. A few years back I would do a normal difficulty run, grab some easy achievements then play multiplayer. Now I don't play on LIVE competitively so much, I focus more on single player achievements and completing games on their hardest diffculties.

I've not played it but supermeat boy looked incredibly difficult. I'm sure there are some hard ones connected to that.
Super meat boy is insanely hard even normally. Some of the achievements like doing a whole world without a death are just evil. But it is a fantastic game
