Did you ever play Project X 2? I was hardware manager at Team17 when this was in development, and the damn Ocean playtesters took over my office for several weeks before it shipped. By then I had had more than enough of it so never actually played it. I'm told it is harder than the original...

GTA San Andreas actually, you were doing the 'Zero' missions.
Definately Vice City, it was one of the missions from the bloke in the Limo. (Forgotten his name) I'm still stuck on the Helicopter School mission in San Andreas where you have to fly round and destry the trucks then land within a time limit. Sooo frustrating, I either don't make it in time or crash when trying to rush back. 
No fighting in the war room and the epilogue on COD4 on Veteran.
Luigi's Purple Coins on Mario Galaxy.
Loads of the platinum medals on PGR2, but notably the cone challenges in the BMW M3 and the Grand Touring class.
Just a few that come to mind that involved a good deal of fist-pumping and yelling when completed.

Ahh, that game brings back some good memories, took me a while to figure that one too if i remember, i must have been about 14 years old at the time. Good taste you have
[i had the amiga version]The last few races in Rage Racer with the stupidly souped up space-age cars was fvcking nails, nearly induced a nervous break down trying to win those (i broke one of my PS joypads in fustration).

Shooting down the attacking RC aircraft on the roof of Zero's store in GTA San Andreas. Still haven't done that one.
The TVR Griff and the Viper in the original GT licence tests. Many hours lost on those.
The final level in RC Stunt Copter. Such delicacy required on the controller but having to be quick at the same time.
ETA: Getting a new character started and past the "dragon food" stage in Moria.
