How on earth would you do this!?!?

They've definitely used elements of photography in there. That building back right isn't hand drawn.
Someone is a dab hand with a graphics tablet!
All the drawing will be done in Adobe Illustrator and the final image is assembled in Photoshop with hundreds of layers etc. Very impressive. Don't like the woman's shoulders though. I imagine it's supposed to be fire but it looks like she has ginger hair on her shoulders.

I used to work with artists who did stuff like that. Absolutely gobsmakingly amazing to see what results they could get with a 64MB Pentium 200 (seriously) and about 500 tons of experience and good old talent, with a side helping of hard work. Some of the stuff in there looks like it has been rendered in 3D with atmospheric effects and HDR.
If you like that sort of work, have a look in the forums here: http://www.cgsociety.org/
Quite often an artist will explain how they did such-and-such.

She also has the cuffs to match!Wonder if there's another few layers for the "alternative ending"!
