Engine rebuilds

I've seen lots of references to TIV's that have had, or need, an engine rebuild - usually the flat 6. I'm no engineer, but could someone explain (in Janet & John terms) what exactly is being rebuilt - and how the rebuild differs from the original?

Also, once a car's had a 'rebuild' how much more likely are they to be more reliable?

Errr....Straight Six, for flat six see Porsche or Subaru.
