Some of the laquer has peeled off on the Mrs car door, one of the 'spots' is the size of a tea saucer. The paint below is completely unnafected. I don't know much about bodywork painting techniques, but is it possible to just remove all the laquer and respray with new laquer, or does the whole lot, including paint have to come off?

all the clear coat has delaminated its self from the base. The clear coat needs to come of and re prime the door. Some times you can feather the edges back on the clear coat ,and prime that aria.Cause leaving the base to long before clear coating. No need to remove base ,but the time you have d/a the clear coat of ,you are going to remove some base.

As above really ..... you will need to feather the edges of the clear, by the time that has been done the chances are you'll have damaged the paint. So it will probably need a quick spray of colour, you should be able to get this done as a localised (SMART)repair and then it's case of re-lacquering
