v5 description incorrect

Lots of vintage/older cars have been modified into " specials " - typically now open topped cars based on what were originally saloons. I understand DVLA isn't happy with changing the description on the V5 and I've heard of V5's being with held or q reg's being allocated.
What are the issues with just keeping a v5 as is rather than reporting the change ??
My understanding is that they don't/won't change the make/model description but you can change certain details on the 'section 1' part of the V5, colour, CC, fuel type, no. of seats etc.
For example my old V8 Land Rover's V5 has it's make/model description as 'Land Rover/88" - 4 CYL' which it was when built but now shows the Cylinder capacity as that appropriate for the larger engine (and nowhere in the details does it specify number of cylinders) so I don't think it really matters what it says in the description.
I suppose if the changes are so great that it's no longer the vehicle it was then it likely needs to be re-assessed and then likely to get a 'Q' plate?
