Oliver Reed features in many such instances.

One unfamiliar to UK TV watchers was an appearance on Ireland's "Late Late Show" in the early 80s. He was - as always - well lubricated but fairly low key that night until the second guest appeared, actress Susan George. Ollie and Sue were old mates so they started chatting to each other. Suddenly Reed leapt up, grabbed George, pulled her off her chair and dragged her behind the set and proceeded to leap on her. All that was visible were two pairs of legs.

Of course, Ollie was only joshing but it was so out of the blue that the host, Gay Byrne, and the audience, were totally shocked.

Can't seem to find it on youtube but that Bridgette Nielsen interview on fantasy football with baddiel and skinner during one of the euro/world cups was one of the biggest car crash chat show moments I can remember. She was absolutely out of it and coming on to frank in a big way. Hopefully someone can track it down because I could do with a giggle.

Not really a chat show, but I think Piers Morgan on Have I got News for You was about as bad as it can get. I distinctly remember pausing it (back in the days of VHS recording) to go and get some booze.
