Smith ended up seriously ill in hospital in 1999 after consuming more than 50 Nurofen tablets, the culmination of a growing addiction to the pills. He said at the time that the pressures of film work were a contributing factor, along with a desperate need to ease the pain caused by gout. Partly as a result, he agreed to sell Talkback, but Smith admitted to restlessness: "I said to my wife that I needed to do something to get me out of the house".

ETA: I doubt whether 50 painkillers in 1999 is the answer; he's been fine since then. I vote stroke. Brave man for going on TV with it though, whatever it is. No wonder he got a bigger than usual round of applause.

Mel Smith has criticised the BBC for not explaining that his strange performance on Celebrity Mastermind was caused by an illness.
The comedian, who appeared to be slurring his words on Wednesday's show, explained that he had a problem with his throat.
"My throat specialist feared I had cancer," he said. "I spent three weeks in agony in hospital. It turned out I had a virulent throat infection and I'm now OK. It was disappointing the BBC would not give an explanation."
A spokesperson for the BBC this week disclosed details of the 56-year-old's condition as concerns about his health grew.
"Mel did it for a charity and didn't want to let [viewers] down, despite not being well," said the representative.

PD will affect everyone in different ways, its a "boutique" disease. Speach impediments can be a symptom, as can problems with the throat (swollowing in particular). Other symptoms are indeed related to facial expressions, no so much total facial paralysis just that facial muscles are affected to the point the expression can look blank.
Its a terrible disease and will almost certainly get worse with time. How qucikly this happens is difference case by case.
