CRB Disclosure Form Help

Afternoon everyone, I've been asked to fill a Disclosure form in for my new job, but I wondered if anyone can help me...

Where it asks me to put my address, it is my parents' house, but I've lived at two other addresses in the past 5 years due to being at university. Do they want me to fill another sheet of A4 with the dates of every time I moved?

A bit long-winded I think, should I just put my parents house and leave it at that?

Not every time you moved but would be useful to put all 3 addresses if possible

When I did it, I think it allowed for more than 1 address......?

Did you get a guidance sheet with the form?

If it wants all the addresses you have lived at in the past five years then list them all. Put the dates next to the address. Might be easiest to say that you were at uni from date - date living at x address and returned to parents at holiday times.

Put your parents house down first, with however long you've lived there

Underneath just put the halls address and the uni house, and put something like term time 1 sep 07-1 jul 08. and same for the other?
Might be the easiest way to do it

I think you should be okay with your parents house. Where were you registered to vote?

I didn't keep all of my bills at my parents' house, or I probably would.

The form gives space for one extra address and only space for a 'date to' and 'date from' for each address, meaning I'd have to list each address for each period that I lived there.

You're probably right there, the advice sheet doesn't really give you any advice, just points out the blindingly obvious. It also says that address irregularities are the most common reason for rejecting an application.

Yes, thats the one. You don't want to miss out on anywhere you have lived.

I would put down your parents house then if I were you. It will save any ambiguity arising as they may not find you in your temporary residences unless you have been in trouble with the Police.

I just put down my main address. If I put all the places I stay for weeks on end then I wouldn't have enough paper. As a general rule if you pay the utilities etc put the address down if you don't ie Halls or Hotel, temporay lodgings, don't bother.
