Over the weekend I gave my S1 Elise it's first real hard use in my hands and now have a clunk on sharp acceleration (ie sharp throttle on)from behind my right shoulder. Could be the seat belt reel but any other thoughts gratefully received.

Hmmm... there's not a lot behind your right shoulder other than the engine and fuel pipe.

Perhaps your engine is about to fall out?

Seriously though, is the sound from within the cabin, or from the engine bay?

If your car is an S1, then you could take the rear window and rollbar cover off and go for a spin and see if you can track down the location. Are you sure it's nothing loose in the cabin, or in the netting behind the seats?

Thanks -- checked the engine and suspension -- both appear well anchored. Maybe the window itself? I'll also get someone to come along for the ride.

Could well be the window itself, the foam strip along the bottom may have come loose and it's moving around.
