Best way is to find out what evenings your local club trains then pop up there for a beer and introduce yourself (or even pop up on a Saturday and watch a couple of games), say you are looking for a 3rd team/Vets (age dependant) game to ease you back in. Do a few training sessions with the lads, then take a brave pill and stick your name down for a game.

Small clubs are always on the lookout for returning players. Just be prepared for how much you will ache after the first couple of games

Read the Local papers, you don't want to try getting into the local top of the division club if that means spending your saturdays on the bench. The RFU site has a search for your local teams function. Don't forget the days of the front row walking from set piece to set piece have gone, so a little fitness is required.

Where in the Midlands are you ? There are hundreds of clubs about so if you let us know we can probably point you in the right direction.
I used to play professional rugby but after a bad injury was forced into early retirement, however I did have a go at social rugby and it's surprising at how much fitness you'd have lost and also how much more it hurts when you've not played for a while! Plus my first match back involved me breaking my ribs scoring a try
I'd recommend perhaps doing 50 situps and pressups a day for a month and maybe running 2-3 mile a day and building it up weekly. Once you feel comfortable with that look at getting into training with a local team (I know that both Stourbridge and Moseley have a "recreational side" with Stourbridge having the larger setup) and take it from there
