Headset Loosing up!

Managed to do a DIY maintenance on the Headset/Fork by stripping and then cleaning the bearings.
Put it all back to together just like Park Tools and Global Cycling Network videos; so all good there.
Now, I've been out on a few rides and keep noticing the headset becoming a little loose; mainly noticeable when braking on a decent or riding over a rough patch on the road; I can feel the vibrations.
I've re-tightened the Headset a bit more, but same thing happens when I go out. I've checked the brakes and these are fine.
I can clearly see and feel the movement of the fork on the steering tube (the spacer and the steering tube) after it has loosened up..
Any ideas? New headset?

If it's a carbon steerer then take out the fork bung, clean it up and refit using some carbon assembly paste. If it's an aluminium one, then check that the star fangled nut is seated squarely - and fit another one if it's not.
All assuming that the stem bolts are torqued-up as they should be.
When it's right they steering should be smooth, but there should be no discernable fore/aft play if you pull the front brakes on and rock the bike backwards and forwards on a hard surface.
One other thought - is the caliper correctly fitted/torqued?
