It depends on whether you have indefinite leave to remain in the UK or not. If you have time-limited leave to remain in the UK, you do have to be checked by your employer every 12 months. If there are no restrictions on you staying in the UK, 12-monthly checks are not required (one initial check at the start of employment is required).
No, that's right actually, what they're asking. Has to be a valid passport (ie not expired - appreciate that seems a bit silly)or (one of the) other documents on 'List A' is the full birth certificate. Once they've seen that once, you shouldn't need to produce anything again.
