Health disclosure for new employer.

I've been asked to fill in a questionnaire for a job as bank secretary staff. It includes height, weight, access to medical records, GP details, vaccination and mental health.
It all seems a bit invasive but will happily fill in if needs be.
Has anyone come across such digging questions for work? (outside of specialist areas where it seems necessary)
The only hesitant bit is the storage of said info and also not really wanting to declare mental health issues which are well in the past and don't really need bringing up as not an issue anymore.
Seems a bit odd- have never been asked such things and is disclosure of all this stuff necessary?

It should only be to make sure you are fit for the job and that if their are any issues that they can support or make changes to help.Depending on anything they found they would not be able to discriminate.
8 years ago I joined my current company and had to do the same and had the same concerns as I had some health issues - no problems and it was ore for the company pension and life insurance than anything else.
My wife had a similar one a few years ago and she had been off with stress for a few weeks with her previous company, they asked her about it and aske if they could help in anyway and said to make sure that if she felt she was getting stress again to make them aware and they would help, also asked if she had developed some coping stratagies.
The other point is that you cant have the job if you dont complete the medical form anyway.

perfectly standard OCCUPATIONAL HEALTH questiosn for an employ er who actually has OH ... It all seems a bit invasive but will happily fill in if needs be.

the appointing manager will not see this, HR won't see it unless you give it to them not in an envelope... it;ll be reviewed by the OH Nurse and one of 4 answers returned to HR
1. no OH issues -
2. can you ask them to come and see us
3. may need reasonable adaptations
4. probably medically unfit to for role
and if they go for option 2 - you'll go and see a OH Nurse or Doctor or perhaps a Physio ( if it;s a ergonmics / back pain / WRULD) and they'll do a report which has one of three outcomes - no adaptations /adaptationsneeded / unfit for this role
