Getting more and more impressed with this body
http://www.raib.gov.uk/publications/current_invest...
Currently it's mainly reactive
Hopefully its reports work for prevention.

I've read a number of these reports and am also highly impressed. Getting back to the days when accident investigations were undertaken by inspectors who were all officers from the Royal Engineers!

yes
It's too easy for some people to say
aargh the rails are suffering from cyclic top, best close all railways
Instead see what's happening, work with it to resolve it

I notice that page mentions work on container attachment as (yet again) a container train has lost one of the boxes. This report - http://www.raib.gov.uk/latest_news/news_archive/ne... - makes for interesting reading. It does make one wonder what was wrong with using twistlocks to attach the things.
