A serious train crash in Southern Italy today with at least 10 deaths . On a single track line looking at the photo.

Driver or signalling error? No doubt we'll find out in due course. Interestingly, the last single line head on collision in the UK resulting in deaths was in 1994 (Cowden) and before that (apart from on single lead junctions) was 1921 (Abermule). There have been several elsewhere in the world over the past 20 or 30 years.

I work in an area of engineering which is closely related to railway safety (same group of standards). Its very rare for a failure to be attributed to pure random failure of instrumentation, signalling or equipment designed for emergency intervention, generally they are incredibly robust and have a large degree of redundancy.
The more common reasons tend to be human-factor related i.e. mal-operation, SPAD, poor design or incorrect initial specification / installation (although in this instance probably not).
