We've made a few.
My company is a stainless steel fabrication business, where our primary market is the food, pharma and biotech sectors.
However, due to the fact that we have a workshop full of equipment for cutting, drilling, welding etc, we can pretty much make anything.
We've done systems based on/in to worktops, modular cabinets, mobile etc etc.
Incorporating refridgeration, cooking and washing; it can all be done.
There are a few companies around who specialise in this type of thing, but they actually make nothing themselves. they end up coming to a fabrication company such as ourselves.
Stainless steel staircases, stainless and glass table tops set into tree stumps, etc etc.
Imagine it and we can probably build it for you.

My brother-in-law has an outdoor kitchen (complete with wood fired pizza oven) as part of his Boma beach bar, which is 30 metres from the Indian Ocean. His is thatched with electric shutters to keep the weather out. He's got marble worktops (prep area), sink, beer fridges, 6 burner and electro-mechanical spit over a deep charcoal braai. I've used it many times over several Christmases and at big family gatherings, and its a real pleasure to use, so I can see the appeal.
If I was going to build one here in the UK I'd be going for stone or stainless steel work surfaces, definitely stainless carcasses for the units, probably used catering grade items. Depending on the size/shape of the area being used I'd ensure a decent sturdy wind break, and of course a fixed roof is a must. If you are leaving things out that will suffer in the elements, then high quality covers fro everything.
Other things to think about:
Electrics, I'd run a separate spur to an RCD and buy the highest quality waterproof switches and junction boxes along with with shielded cabling - everything must be earthed. I'd also be looking at high quality sodium/gel lights to illuminate things, used film lights off the 'Bay are a good shout.
I'd stay away from mains gas outside, and go for gas cylinders.
Water, I'd ensure that a mains pressure feed is linked to a sink unit with appropriate drainage.
Anything else, give me a shout.
