Food hygiene rating of 4.... What does it mean exactly?

I suspect that the corporate places that get 5 out of 5 (such as office canteens and chain restaurants) mainly do so because they can whip out a folder of process documentation when the inspector calls rather than being any cleaner than a '4 star' establishment that might actually be concentrating on the quality of food rather than the brand of worktop polish.
