They made me my custom blanks.
I have three custom sets, one 'normal' with -25 filters, one with no filters, and one full concha with no filterse (kind of like an in ear monitor but all solid).
All made from the same single mold. Useful to have a choice for applications, especially planes.

The best off the shelf I have found are the pure wax ones by www.quies.com.
I tried foam, foam-mousse and other wax brands (but some are sticky and disgusting). They will not cut sound by 100% but as they soften inside the ear, they can be spread and pushed further thereby creating a decent seal.
I don't have an auditory problems except an occasionally noisy sleeping environment. I find they are excellent and available from many pharmacies.

I've two sets - one ACS cheap set that take out enough to stop ringing (my band practices louder than we generally gig!).
I've also got a set of silicone ones that are moulded to the ear and block too much to play with - they're formed from two compounds you mix and then mould to fit your ear exactly including as deep as you like into your ear. they then 'harden' to nice soft silicone. Not completely noise free but comfortable and block a lot.
If you've a big budget ACS stuff is great.
