I've been using shure 215's for a few years and I'm generally happy with them apart from the fit, I get a lot of leakage and poor bass unless I have a brand new pair of large buds on and shove them right into my ear.

I have a pair of KZ ZS10's which people rave about which are no better than the shures.

I've come to the conclusion that the issue is my ears and so thought I'd get some custom ones made.. I appreciate they are expensive.

Anyone got any recommendations ?

Have you tried Comply foams? They’re much better than the standard tips, and almost as good as custom fit.

No but I'm looking now, do you use the normal ones of the longer'professional' ones?

I prefer the normal ones, the longer ones are too long for me. I’ve used Shure SE’s of various descriptions for years (I’m a film and TV sound recordist, and sometimes it’s necessary to not wear over the ear cans) and I’ve found I prefer complys to custom moulded tips, I’ve had both custom moulded IEMs and custom moulded tips for the SE’s and I’ll use Complys over either,

Most people use either Cosmic Ears or ACS. Neither are cheap.

Edit. Ultimate ears too.

These are much better than standard, although they sent me the wrong ones for my 215's, luckily they fit my cheapo amazon ones. The seal is better, more bottom end. Gig next weekend so will see how they fare.
