Carers Documentary...

Hi,
I'm currently in the research stage for a Documentary about carers (for terminally ill people). I've found a couple of interesting subjects, but am desperate to stick to my original idea of making a documentary about the carer/s and not the sufferer. I don't want the piece to become a sob story or a patronising account of their lives. I have found one really interesting subject who is a controversial member of the carers community (very outspoken and disliked by many, gets death threats etc.) but just cant think of an angle to come at it from. I know this is vague as non of you will know the ins and outs but was wandering if there were any ideas.
