Shutter Island ****DEFINITE SPOILERS**** a few questions

As this has been out a while now and there is a thread running that doesn't have spoilers I wanted to ask a couple of questions. BELOW HAS SPOILERS YOU HAVE BEEN WARNED.
Who thinks he was insane? Why?
Who thinks he wasn't? Why?
Now regarding the last line which was something like "is it better to live as a monster or die as a good man" I understood it in regard to everyday life but not in the context of the flm. What was he trying to say?
I had the plot sussed by the time they had supposedly recovered the escapee lady back to her room as it was a pretty obvious plot not because I had cleverly worked it out.
Any more thoughts

Very good film, and kept with the book well, great use of sound, characters were great.
