According to this story. Patient died after being given drugs he was allergic to. The only person disciplined so far is a nurse who raised concerns.

Not surprised. My MiL is currently in hospital at end of life. She had correct surname, wrong first name above her bed. if they can't get the basics right, what chance.

Not surprised either. My mate had the wrong leg amputated, and he only had one to start with.

I've not read the details, but it's entirely believable


As is the inevitable witch-hunt against whistle blowers, despite a whistle blowing policies and their protection being enshrined in law.


Yes, we don't spend as much as other countries on health, and generally sure most NHS workers do their best.


But the overall culture allows poor performance to exist and survive. A bit like the police turning a blind eye to racist or violent officers on those whatapp groups. A few "bad apples", but they're bad in plain sight.
