Scarey really, used to be a programme on Sky called "Seconds from Disaster", I remember one when a building suddenly collapsed in Singapore.

Upshot of it was that when you design a building, say it weights 50,000 tonnes, the structure must be able to support that weight PLUS WHAT YOU PUT INSIDE IT.

The design team forgot about this. The structure was not designed to support a fully kitted out building, hense it collapsed. This was a few years after the thing was built.

When the mirror in the bar in the ground floor suddenly cracked, and one of the supporting pillars in the underground cracked some days before, no one thought about it, the cracked pillar was painted over, and the mirror replaced.
