amex (avios points)- paypal payments

hoping some of the knowledgable people on here might be able to help.
in january I am going to be paying about £1200 per month on nursery fees. adding in some personal spending I am easily going to cover the £10-15k (depending on card) spend time get the companion tickets using a BA amex card.
however the nursery we have chosen no longer accepts amex, i’ve heard of people using paypal as a way of still using an amex card when places don’t use/ accept it. or will i end up being charged huge ‘cash advance’ fees and miss any benefit of the avios points?
seems a shame to miss out on many avios points/ companion vouchers, when I have a guaranteed spend on nursery fees

I regularly pay things through PayPal on my Amex and never had any fees. But that’s transactions through websites which allow PayPal as a payment method rather than logging into PayPal and sending money to someone.
