Peely Paint

Help please. I'm redecorating the toilet walls using an emulsion on top of some old paint. Areas are bubbling up and the old paint is peeling off exposing the plaster. I'm removed all the loose paint, do i need to sand off all the old paint or can I apply some type of sealant? Why is this happening?
