Unless what you're proposing is of a contentious nature, I wouldn't say you need a Planning Consultant.
Your Architect would liaise with the Planning Department, prepare supporting info. and submit the formal application.
That's certainly how we'd approach it in our Practice anyway.

Your architect can (should be able to) submit your application. I doubt you'd need a planning consultant though - they can be expensive (but a good one can be worth their weight in gold on a contentious proposal).
Fees are sometime a % of the overall costs, but be careful who you trust on this principal as they can easily over-specify (thus netting themselves a better fee).
You should be able to work to a fixed price on a simple extension, with agreed rates for any 'overs' (to be agreed in writing prior to the 'over' commencing) - this is how I like to do it.
