I need a mechanical 'thing' that can lift a table top 45mm

Someone help me before I go crazy!
I need to make an extending conference table that extends from each end - the centre piece has to stay put.
The main table is 1500 x 1100 and it needs to extend by an extra 650 each end.
Pulling the extension leaf out from under each end is fine but it is then too low so it needs to raise up by 45mm, i have made a wooden scissor lift but its ugly and does not work very easily.
What i want is some kind of worm drive thing that can be mounted on each corner and then raise the top up to sit flush with the main top.

Instead of the extension leaves being lifted up, can you drop the centre section when the extension are in the outer positions?
Closed the centre sits on the outer hidden leaves. Pull leaves out, let centre drop into place. To collapse, lift edge of centre section and push outer leaf in.
Whole lot sits on a square frame under the centre, with the extensions have legs which are cantilever supports when extended.
That's how my parent's dining table worked.

We use these guys for some of our plasma lifts. Speak to Gus. Might be over budget however!

When I was a child my parents had a dining table that extended, the extension leaves each had two long supports made of hardwood fixed to their underside. IIRC the idea was to push the leaf downward slightly then draw it out, then slide it back in so that the supports engaged in slots in the apron of the table in such a way that the leaf was raised flush with the centre section.
I'll think about it some more and se if I can remember exactly how it worked. Simple and very effective.
Presumably the much easier to achieve solution of splitting the centre section and inserting the extension there isn't acceptable?

Cheers fellas - no electrical options were coming in on budget
Although I now have lots of ideas floating about that I might be tempted to make and sell!
The table had to have a fixed central section as it houses the control box thing for the plasma screens/projectors/sockets etc
Finally got around the problem by modifying an existing design and runners, basically a double butterfly table.
Not ideal and had to loose 3" of the extended length buts needs must in this case. But still at 2.9m its a big table!
