You still enter under the visa waiver programme, they give you the green card to fill out on the aircraft before you land.

You also need to complete an online ESTA form at least 72 hours before you travel - https://esta.cbp.dhs.gov/esta/esta.html

You still need an ESTA however you no longer need to fill out the Green Card that they use to give you on the flight prior to landing, I was in the states last month and was told that the ESTA has now completely replaced the old system of having to fill in the green card.

You dont need flight details or dates for esta

You will fill in the advance passenger info with the airline, which consists of flight details and address of your first nites stay.

just back today from NYC. I had to get the ESTA $15 and had to complete a white form on the plane (1 per family)

That's the customs declaration form which you've always had to fill out. But with ESTA you no longer need the I-94W (the green form).
