Travel Insurance with pre-exisitng conditions

We're driving through France and Italy for 3+ weeks in September, and looking for reasonable, reasonably priced travel insurance. I've had a heart attack (2 yrs ago), and touch wood everything OK since, and Mrs Rupert has a lower back problem. Has anyone got any good suggestions for insurance, because some of the quotes that are coming up are just silly. (Sorry about typo in title)
