Passport Confusion

Right long story cut short, I am wondering how do I obtain an adults Passport? The only previous Passport I have is an expired (5 years) child's one, so what on earth do I do? I've tried internet searches and the like and am still none the wiser. I am hoping the PH collective can shed some light on the matter and relay the information in simple terms, any information would be appreciated. Thanks

I think it as simple as picking up a form from the post office.
