Don't pay an annual membership, stick to monthly to be paid in cash/cheque and not direct debit.

. If you're looking for etiquette advice:
-Take a towel, always wipe equipment down after you're finished with it.
-Don't sit on one piece of kit too long unless there's clearly nobody nearby looking to use it.
-Don't watch other people too much.
-If someone asks you to spot them on something, then it might be best for you to explain that you're new to the gym and won't be much help.
-Generally, people will be happy enough to help you with things if you ask. If you're doing free weights, asking the big muscley guys for advice may help (it -probably- won't result in mocking etc), but always take what they're saying with a pinch of salt.
-Gyms are very bad places for the spread of knowledge - so many opinions with no basis in fact will be given to you as fact, and many of the people doing this are passing on what they've been told by others rather than what they've actually researched.
Practical considerations (though these are more obvious):
-Loose fitting clothing and trainers.
-A big bottle of water. Better to have too much with you than too little. I usually take 1L for a short weights session (for cardio I'd take more).

get a trainer to show you everything and get a plan. dont wander round doing a bit this and a bit of that! you are there for a purpose..to get fit and stronger...even slimmer. Do not spend longer selecting a tune on your ipod than actually using the machine! Watch people who are better than you for technique and tips on how to improve!

First set and rep- heavy as possible.
If some one is doing 20kg on a machine/dumbell, do 21kg+ even for 1 rep, and don't be afraid to put your back/knees into it!
Grunt and groan every rep- like your balls are caught in barb wire (face wise).
Where a "Golds Gym" muscle vest.
Use chalk on your hands.
Drink a sports drink/protein shake whilst training.
If there is a punch bag, do a 10-20 sec blast of punches.
Do one rep, and then check for progress by posing in the mirror.
Stay TOTALLY focused- iPod on, and act like you are pulling the kids from a burning house.
Do all of the above and you should blend in fine in most gyms

On a more serious note...
To build muscle, is it better to do more reps with less weight or less reps with more weight and, in the case of less reps + more weight, how many reps is the minimum before it's too few to be useful?

I see people like this all the time!
The one rep and check progress in the mirror is what I see the most.

I recently joined a small local gym attached to a hotel. There is a realllllly small weights area surrounded by mirrors (I don't really do freeweights at the moment). There is always some orange jersey shore reject standing there in front of the mirror wobbling all over the place trying to lift what appears to be 50 tonnes on each end, and failing miserably. But still nodding at himself in the mirror and looking "well 'ard"
