ENG1 Medical Exam

Went for my 2 yearly ENG1 medical yesterday - Merchant Navy requirement for seafarers in engineering and deck capacity.
All goes well, doc going through the checklist and, as usual, I disclose the fact that I have Colitis (30 years since diagnosis) but only occasional flare up lasting a week or so perhaps every two years so well contolled with medication. Same as I tell all the other doctors that have issued the medical with no problem. This one though decides to restrict my medical to near coastal only (150 miles). I ask why and he says I may have a violent flare up leading to a perforation! Now in the last 30 years, I have never had a violent flare up, I believe this is more symptomatic with Crohns than Colitis......
I ask him to show me his guidelines for this........there are none i.e. no restriction required. He says he will consult with Chief Medical Examiner for advice and gets back to me today saying he agrees with him and restriction will be placed. Yet for the last 20 years I have unrestricted medical i.e. worldwide use of my Deck Officer ticket.
Am I right to be pissed off? I am going to appeal obviously but wondered what PH knowledge base thinks to this?

As part of your appeal process I would recommend that you have supporting paperwork from your specialist who manages you for your colitis. A supporting medical letter would then be able to verify your long established history of good control and if it's the specialist be able to quote evidence based statistics around any emergency complications that would require acute hospitalisation (I presume you carry any medication required to manage mild flare ups). You might find the NICE guidance a useful read in terms of looking for supporting evidence base, pdf version available on the web address below.

It is very strange that you've been given an offshore limitation. I've only ever heard of an annual restriction of an ENG1.
