Limb power meaning ?

I have received a copy of a letter sent to my GP after being assessed by a doctor at a rehabilitation centre due to continuing problems after a stroke.

It states I have power 4+/5 in the left upper limb and right lower limb. Power 3+/5 in the right upper limb and 4/5 in the left lower limb.

I have no idea what the above means !

Can anyone explain - ie, what is normal etc.
