Neck and shoulders making strange noises

My neck makes a terrible grinding sound whenever I turn my head, it sounds like gravel being churned about. I also have alot of shoulder pain all of the time partly due to poor posture and partly due to having the neck and shoulders of a 12 yo girl.
Has anyone else had this? What's the best first port of call the gym, doctor or chiropractor?
