Finger nail question for Doctors

About 9 months ago I tripped while carrying a hammer which resulted in the nail prying end of the hammer smashing into my right index finger nail and the skin immediately below it. It was a mess.
I saw a hand surgeon who told me to let the nail grow out, fall off and get regrown, which it now has. The only problem is that the nail is heavily dented and continually splits at the end.
Is it possible to resolve this with surgery? As I live in the USA I will be paying for it so I would like to have a clear idea of what is possible and the success rates before talking to a surgeon.
