Pilonidal sinus not healing.

In November I suffered a flare up, and had an operation on my right buttock, it was left open to heal from the inside out, totally text book. All done and finished with.
However, 8 days later, also in November the left buttock flared up, and was operated on, no track to the one on the right was found. An M.R.I. scan was done, nothing unusual found. But this failed to heal properly, and on new years day the district nurse wasn't happy, and sent me back to hospital, I had to have a second op, as the skin was starting to heal, but puss, and some blood were still discharging, they just made the same hole bigger. Move on to 2 days ago, and exactly the same issue with puss, and blood, again the same area operated on, and the hole is now massive 5cm x 7 cm according to district nurse today. I have been issued with no antibiotics this time.
Has anyone else had such a bad issue trying to get one to heal. It is very close to my sphincter, so walking, and going to the toilet are difficult. And I haven't been able to sit properly since November, I either lay down, or sit on my hip and elbow on the sofa, and quite frankly I'm fed up of it.

I've had one for the last 20 years or so, just about manage to keep it at bay by sitting carefully
I've often thought about getting it fixed but it's never been quite bad enough to warrant such a nasty op

I've been showering once a day, will try more often.
As it gets better, the nurses change to dressing it every other day, and ask my wife to do the other days, but she struggles to pack it properly, so that may be an issue.
I'm a non smoker, and letting no creams at all near it.

When wounds don't heal, it is usually due to "bad" blood.
If you are fat, have prediabetes, or vascular issues, then you need to fix those problems first. Fix the holes second.

Ironically when I was abusing my body with sugar, and fatty foods, I had no issues, start looking after myself, and dieting, all this kicks off.
And yes I know it's not connected to my lifestyle changes, just to my old, bad choices.

Usually on people who have had traditional pilonidal sinus disease procedures which have failed to heal or have developed further disease - I have a 0% recurrence rate.
The condition is usually treated by General Surgeons - the diseased tissue is cut out and a large hole is left to heal by secondary intent.
As a plastic surgeon I do things differently - namely excise the diseased tissue using a dye to highlight the extent of the subcutaneous extension. I will then use a flap of tissue (skin/fat/fascia) with its own healthy blood supply and rotate this in to provide new, healthy tissue to fill the hole. Once healed I then perform LASER hair removal in the surrounding area to further reduce recurrence.
This is an extremely simple technique that plastic surgeons use to fill defects all over the body - I'm still amazed that this technique is not used more often.
I would recommend to anyone suffering with PSD that you ask for a referral to a plastic surgeon (we don't just do facelifts & breasts !!)

I’d always wondered around laser hair removal as my hair is dense and coarse. Would you recommend it? The big fear is another
You are quite within your rights to ask for a referral to see a plastic surgeon. Not all hospitals will have a plastic surgery department but pretty much all the big teaching hospitals will have one and will often run clinics in the smaller hospitals.
As for hair removal - given that one of the primary causes of PSD is hair related it makes sense to try and remove the hair. I recommend LASER hair removal using a LONG PULSED ALEXANDRITE Nd:YAG or DIODE LASER (avoid IPLs) - it works best on dark, coarse hair.
