Only thing that will tell you one way or another is an Xray or MRI.

Everything else is a guess.

Well not really, a decent physio should be able to tell you where it's muscular or not.

Unlikely. In fact quite the opposite, they might both be normal.

An experienced assessor and a skilled examination more likely.

I was talking about arthritis, sorry should have been clearer.

Could be anything.

Arthritis.
Impingement.
Tight muscles.

Not sure a physio or doc or anyone else for that matter could tell you for certain otherwise they wouldn’t rely on medical imaging as much as they do.

I damaged my back 20 years ago - grand piano related, but that’s another story. Anyway, having visited the doctors, I was given painkillers and physio. It helped, but what really helped was seeing an osteopath. Over the years, it would let go, another trip to an osteopath and all was good. It’s now absolutely fked and having had an MRI recently, the damage was clear to all - my point being, get an x-ray or MRI now, which I didn’t at the time.
