Continuing Healthcare funding application

I'll try to keep this short. My 90 year old uncle is in the local hospital after falling at home, he has stage 5 Parkinson's disease and Dementia. His wife has recently passed away, they have no children and he needs to go into a nursing home. My wife and I are next of kin and are in the middle of the Continuing Healthcare funding process and are looking for advice on how the Decision Support Tool should be correctly filled in and how to establish there is sufficient evidence to best support the application. His application was sent back by the hospitals internal quality control due to lack of evidence in certain areas.
He is predominately bedbound, requires 2 staff to reposition him and a hoist if he requires lifting, he is resistant to the help offered. His legs are rigid and crossed and he suffers pain when he's moved. He frequently refuses or spits out his medication. He can not reliably feed himself. He suffers from hallucinations, delusions and only on a few occasions is he in the real world, he is generally very confused.
He owns his own home and is over the financial threshold.
Any help/suggestions would be very much appreciated, we want to do what is best for him in a very sad situation. Thanks.
