"perform an intervention and get him into a program by force"

No adult can force another adult into treatment against their will, unless they do not have Capacity to make a decision about their wellbeing. EG a psychotic patient does not have capacity and will be held under a section of the Mental Health Act. An unconscious patient will be treated usually by us, in their best interests, whilst we know of no pre-injury direction against this.

Disregard for your own health and slowly accelerating your death by the lifestyle you describe, is not a loss or lowering of capacity.

The patient will need to agree to the change, unless there is a defined/assessed underlying mental capacity issue.

Also this book was a bit of an eye opener for me. The Obesity code - by Dr. Jason Fung (loads of presentations by him on Youtube)

https://www.amazon.co.uk/Obesity-Code-unlocking-se...

His theory is that obesity is nothing to do with calories, it is solely down to hormone imbalance mostly Insulin & Cortisone. The main problem being processed foods, sugar, and the promotion of low fat diets since the late '70's. He is also a big fan of fasting, Intermittent fasting / OMAD (One meal a day) over a low calorie diet.
