Sorry I can’t answer your question directly but has there been a discussion with the GP or specialist about whether warfarin remains necessary? For the most common indication for anticoagulation (atrial fibrillation) many people have been encouraged to switch to one of several newer anticoagulants that require no monitoring, no dose adjustments and don’t require caution with what you eat and drink. The switch is not appropriate for everyone on warfarin, mind.
