"Nourish" care home record system

Does anyone have any experience with the "Nourish" record keeping system used in care homes, specifically the care plan side of things, and who wouldn't mind answering a couple of questions for me please?

My mother is in a care home and I think the care plan looks a bit iffy, but I need to clarify something before I can be certain.
